You have a choice of front, Blockbuster or Blockbuster Video.

On the reverse, using a font from the era, your choice of details including joining date, store location, member name, and membership number.

Fill in the details you want on your card, in the personalisation boxes.
You must give Name, Location and Issue Date to go on card.
Store Phone Number and membership number to go on card are optional and will be made up at random if you don't enter them.
